The World Bank Group's life insurance provider, Prudential Insurance Company, offers HQ-based staff who are not eligible for Retiree Life Insurance upon termination of employment the option of enrolling in the Prudential Portability Plan.

01	Locate the Life Insurance Portability certificate that was provided as part of your personalized Ending Employment package.
02	Use Guide - Prudential Portability Brochure to help you make a decision on the coverage you will apply for.
03	Complete these forms and submit to Prudential using the address indicated on the forms (along with the certificate described in Step 1 above): Form - Prudential Group Term Life Insurance Portability Election Form - Prudential Short Form Health Statement Questionnaire Note: Portability is a Prudential life insurance product. The Bank Group is not involved in any aspect of your coverage.
04	Receive your enrollment confirmation from Prudential.
