Medical Underwriting for HQ Staff – Optional and Portability Group Term Life Insurance or Optional Dependent Group Term Life Insurance

HQ-based regular, open, term staff, and executive directors are eligible for Prudential's Optional or Optional Dependent Group Term Life Insurance upon being hired by the World Bank Group. This process is applicable when:

a.    a staff member fails to enroll or have not elected their life insurance option within 60 days of entry-on-duty or upon a qualifying life event such as the birth or death of dependent(s), divorce, marriage, legal adoption, establishment of a domestic partnership, or dissolution of a domestic partnership.

b.    if the staff member chooses to continue with the life insurance with Prudential under portability benefit.

In both cases, they will still have an opportunity to enroll within the 'Late Enrollment' policy/guidelines.

To submit a "Late Enrollment" the staff member will need to complete an "Evidence of Insurability (EOI) Short form" signed by the staff's manager. Once submitted, Prudential will review and either approve the request or request additional medical information which would be provided using the "EOI Long Form"

Decisions to approve the optional life insurance coverage is a decision solely made by the World Bank Group's insurer. The Bank Group has no influence or control over Late Enrollment or portability decisions and cannot overrule the decision communicated by the life insurer.

Access myHR Self-Service. Use the following menu path: Statements and Letters >> Letters >> Employment Verification Letter.

Select the Prudential Evidence of Insurability Letter and elect the increased level of Optional Group Life Insurance coverage you wish to apply for.

Print, complete, and sign the Prudential: Evidence of Insurability Late Enrollment Short Form and Top Sheet.

Mail the completed form, top sheet, and Prudential Evidence of Insurability Letter to:

The Prudential Insurance Company of America
Group Medical Underwriting, P.O. Box 8796
Philadelphia, PA 19176
Or fax the completed form to 877-605-6671

Receive the decision from the Bank Group's life insurer, which may include a request for additional medical information and/or the long form to support your application.

Some late enrollment applications require urinalysis and blood profile results performed within the past six months. You may be required to submit results of these tests to the life insurer. If you have not had these particular tests in the past six months, you may need to have them done at your expense.
