Marina Mahathir, I appreciate your sentiment, that people are dying while we are discussing economic issues. However, ignoring the economic limitations associated with HIV/AIDS treatment won't lead to any less suffering or dying; ignoring economic realities in the long run only increases problems. The issue you raise regarding curtailed prevention programs is a critical example. Prevention is generally more cost-effective than treatment. Given limited funds available, should the emphasis be on restoring prevention programs or providing ARVs? What would be the trade-off in terms of increased incidence if funds (either government or NGO) were directed towards providing ARVs? The counter to this argument is that all necessary funds should be provided for both prevention and ARV. However, until that level of equity occurs, we economists will have to focus on ways to produce the best outcomes with available resources, that is, to be cost-effective. |
