My main experience is in the United Republic of Tanzania. The health care situation is extremely difficult: hospitals have hardly any impact on the health status of the country; the proclaimed achievements of PHC do not exist; more and more people are seeking help from traditional healers,... Even basic health care is not accessible or affordable for a large number of rural and urban citizens. A first look at the costs of ARV treatment of all AIDS-cases in Tanzania (acc. to our own simulations) shows that this would costs more than the entire health care budget of Tanzania! I see no justification for that as long as thousands are dying of malaria, TB, etc. We should stress health reforms, re-allocation of resources, community based insurances, hospital rehabilitation, management improvements, ... When all this has been achieved ARV treatment can be implemented.
